Specialized Support Services Coordinator





This vacant position requires working with clients with complex needs, providing comprehensive supports that includes crisis intervention, housing assistance, system navigation, and case management. The Specialized Support Services Coordinator works collaboratively with the Specialized Support Services team to set up and provide wrap-around services to support client needs. Developing and maintaining a strong working relationship with health and social service community partners is foundational to this position.

Primary Duties


Identify and facilitate client access to community resources + Completes intake assessments using LEO's Standard Intake Form and the Specialized Support Services Intake Form, financial assessment forms and when required interRAI tools
+ Documents and maintains client files in accordance with LEO/program expectations, including statistical entries and re-assessments at a minimum annually or as needed
+ Assesses client needs and develop an appropriate plan for possible housing and wrap around support based on the need of the client and their goal(s)
+ Assists with referrals to secondary and tertiary health care services
+ Coordinates client access to and navigation through the healthcare system
+ Liaises with health care providers and community agencies as required to coordinate wrap around services for the client
+ Advocates for clients to ensure that they receive the community supports, and health and social services they require
+ When necessary, supports clients with services such as errands, attendant shifts to appointments
+ Manages a caseload, plans for discharges, determines priorities, meets deadlines
+ Track and reports active clients and case loads to the Manager monthly

+ Maintains effective communication with other Specialized Support Services team members and those in the client's circle of care

Documentation


Documents accurately in AlayaCare on each client according to LEO's Documentation Policy and Standards of Practice Utilizes the SBAR format when required for documentation




Staff Development


Attends and completes all mandatory training/assignments Participates in relevant workshops and training sessions Has the desire and ability to update knowledge and skills through various means including self-directed learning, technology-based opportunities, courses, workshops, and conferences Participates in the Performance Development Review process

Ensures compliance with privacy regulations Ensures confidentiality and safeguards client information Knowledgeable of LEO's goals and objectives Seeks guidance from the Director of Clinical and Community Support Services when needed Is dependable, courteous, and punctual Notifies the Manager of Specialized Support Services or On Call of any illness, emergencies, or absence Follows LEO's Code of Conduct, Policies and Procedures Promotes and abides by LEO's Team Commitments Maintains a positive image of LEO in the community Understands and follows the Client Bill of Rights Reconciles and submits accurate timesheets, including travel time by the deadline Submits time off requests through BambooHR for approval Utilizes communication tools effectively, e.g., Microsoft Outlook Performs other duties as assigned by the Manager of Specialized Support Services Adheres to LEO's standards regarding AlayaCare and Outlook

Working Conditions


Working in an office environment. The position also involves frequent travel throughout Lambton County to meet with clients and/or caregivers for assessment purposes. Occasional travel is also required to attend training and/or meetings outside of Lambton County. Overtime as required and approved by Manager Dealing with complex clients or situations may be part of a working day. Manual dexterity required to use desktop computer and peripherals

Requirements


Completion of a satisfactory Criminal Reference Check and Vulnerable Sector Screen Post secondary education in social services, nursing, gerontology, or equivalent. Social Service Worker education and experience preferred. Excellent verbal and written communication skills and the ability to communicate effectively with individuals at all levels of the organization Prior experience and knowledge of electronic medical records, especially with AlayaCare is preferred Proficiency in the use of computer hardware and software, particularly with Microsoft Word, Excel, and Outlook
