Social Worker/mental Health Therapist

Job Description
The Social Worker/Mental Health Therapist plays an integral role in the care of patients of the Brockton and Area Family Health Team (BAFHT). Working in collaboration with physicians, nurse practitioners, allied health professionals and administrative staff, the Social Worker/Mental Health Therapist provides services and programs that are responsive to the needs of our patients and communities.
Main Responsibilities:
Provide individual counselling using a range of therapeutic modalities for patients with mild to moderate mental health issues under a brief-therapy model. Conduct intake assessments to ensure that patients are being linked to the most appropriate mental health services, considering both internal and external options. Work closely with other team members to provide a collaborative approach to patient care. Support patients with System Navigation to facilitate access to appropriate health and social services. Participate in the planning and implementation of education and treatment group programs.
Position Qualifications and Preferred Experience:
Bachelor of Social Work or Counselling degree from a recognized program Current registration with the OCSWSSW or OACCPP Demonstrated skills in assessment, program planning, implementation and evaluation Recent primary care or community-based counselling/therapy experience Demonstrated ability to work collaboratively within an interdisciplinary team Excellent interpersonal, communication and organizational skills Experience coordinating patient care using community resources Proficiency in the use of computers and information technology

Contract length: 13 months
Pay: $37.87-$40.53 per hour
