Unity Health Toronto, comprised of Providence Healthcare, St. Joseph's Health Centre and St. Michael's Hospital, works to advance the health of everyone in our urban communities and beyond. Join our team as we work to put patients and families at the centre of everything we do, in the role of Social Worker with our WeSTHC site.


Social workers in health care provide in-depth bio-psychosocial assessments, diagnostic, treatment and evaluation services for individual, family group and couples. Counseling, discharge planning, resource referral, consultation and collaboration, teaching, research and patient/family education. In this role, The social worker will collaborate with an interdisciplinary team and a community of practice of social workers across the family health team's five family medicine sites. The SW will provide direct care to patients with acute mental health concerns via outreach visits to patients homes and within the community.


DUTIES & RESPONSIBILITIES:




Work effectively as a member of an interprofessional team, collaborating with other disciplines to provide comprehensive patient care Contribute to the academic mandate of the organization as an educator, resource or support for staff and members of the academic and professional community Identify and address educational needs for patients and their families Demonstrate a commitment to ongoing professional development in order to continue to provide optimal patient care Are committed to and participates in ongoing quality improvement and/or research in the organization Provide patient care in accordance with all professional and organizational standards Are committed to providing anti oppressive and accessible care for all patients Demonstrates understanding of the social determinants of and knowledge of how to access and link patients to appropriate community resources and services such as: OW, ODSP, Trillium, Community Housing, etc.

QUALIFICATIONS:




Must be currently licensed by and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) for practice in all applicable clinical areas Must hold a Masters Degree in Social Work Current BLS (or willingness to complete within probationary period Excellent communication, critical thinking, and interpersonal skills Proven flexibility and adaptability in a fast-paced environment Excellent time management, organizational and prioritization skills as relates to patient care Demonstrated advocacy, commitment to and involvement with profession preferred (e.g. involvement with professional association, representation on relevant committees Eligible to engage in student education with academic partners. Must be currently registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) for practice in all applicable clinical areas. Excellent communication, critical thinking and interpersonal skills Proven flexibility and adaptability in a fast-paced environment. Excellent time management, organizational and prioritization skills as relates to patient care. Must be willing to work evenings and/or weekends as directed by the Program.
