This position will be for 2 days per week, from September 2025-March 2026.
Summary of Duties
To provide a full range of Social Work services to patients and families in the Short Term Complex Support and Transitional Care Program. This includes psycho-social assessments, counselling to patients and families, discharge planning, crisis intervention, linkage to community resources, and team consultation.
Qualifications/Skills
Masters Degree in Social Work
Member in good standing of the Ontario College of Social Workers and Social Service Workers
Experience working in a health care setting (preferably 2 years)
Experience with the health care system with discharge planning and community resources
Familiarity with issues of capacity and consent
In-depth understanding of the Sunnybrook Discharge policy and its application
Strong negotiation and conflict resolution skills
Ability to be a team player as well as self-directed
Cognitive support experience required
Excellent assessment, interviewing and problem solving skills
Excellent written and oral communication
Superior ability to manage multiple tasks and set priorities in a fast-paced environment
