Carefirst Seniors & Community Services Association
is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area.
With a staff of 500 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are "home-bound" and frail.
We are currently seeking a
Social Worker
to join our team.

300 Silver Star Blvd., Scarborough and community locations in the York Region and City of Toronto
Essential Duties and Responsibilities:
Practice within an interprofessional setting, provide social work services within scope of practice, legislation and best practices in community health setting, including mobile health clinics
Provide complete psychosocial assessment and care planning with client systems, care planning, short-term/goal-oriented counselling and psychosocial support, interdisciplinary problem solving, and liaising with internal/external partners to ensure integrated care and service navigations for client systems
Engage in early identification, response, and prevention of mental health challenges and chronic conditions
Provide health promotion and wellness-based models of care with individuals, families and groups
Jointly develop, implement and evaluate this innovative, community-based model of mobile health promotion and wellness clinic in addressing health equities
Establish and maintain meaningful community partnerships in support of client care and program development
Other duties as assigned in support of operations
Qualifications & Job Requirement:
Master of Social Work (MSW) strong asset
Valid and current registration with the Ontario College of Certified Social Workers and Social Services Workers
Demonstrated current expertise/experience in EDI and anti-racism practices, integrated primary care and management, chronic disease prevention and management, health promotion and wellness
Strong asset to have experience and expertise in community-based mental health care and crisis management
Capacity to travel within York Region and city of Toronto for some outreach clinics. Valid Ontario Driver's license and access to vehicles strong asset
Demonstrated understanding of culturally responsive health and prevention models, health equity, social determinants of health. A strong asset to have experience and expertise in providing integrated health care to equity-deserving populations (such as Black, 2SLGBTQ+, racialized communities and more)
Advanced clinical knowledge and implementation of evidenced based practice
Advanced knowledge, skill and decision making and expertise in the nursing care of primary care clientele.
Proficient with EMR and computer programs including, Word, PowerPoint and Outlook
Demonstrated strengths in interpersonal relations, problem-solving, and conflict resolution. Demonstrated excellent communication skills. Demonstrated commitment to professional development
A valid vulnerable sector police record check completed within one year
