As a key member of Taddle Creek Family Health Team (TC FHT), and an integral part of an interdisciplinary health care team, you will deliver social worker care in accordance with TC FHT policies and professional practice standards. You will perform routine social worker duties, and ensure that the needs of our patients are met.
Your primary roles are to work in partnership with TC FHT's primary care providers & interdisciplinary health care providers to perform patient psychosocial assessments, provide short-term counseling, education/advocacy, case management, and group/workshop facilitation.
You will deliver services to a variety of patient populations including patients experiencing anxiety and/or depression, trauma and other mental health disorders and occasionally homebound patients.
Roles and Responsibilities
Psychosocial Assessments
Review referrals & patient charts
Provide psychosocial assessment for patients and their families
Analyze findings of health assessments and interpret patient health records
Monitor, record and report symptoms and changes in patients' conditions
Collect data through a therapeutic relationship with a patient
Treatment/Management
Provide short-term counselling including instrumental, supportive, psycho-educational crisis intervention, short-term psychotherapy and group facilitation
Coordinate referrals and facilitate access to other health care providers and community organizations
Provide case management
May perform house calls and community outreach visits
Maintain communication with patients and families
Monitor patient goals and outcomes through ongoing assessment and documentation of treatment progress
Participate in treatment planning with other team members
Consult with team members and external professionals involved in the care of patients
Education/Advocacy
Initiate group programs to promote and support patients as they strive to achieve the highest possible level of health
Act as an advocate for patients and their families
Assist in overcoming barriers within health care system for timely delivery of services
Help patients identify and use health resources, including those available in the community and from community agencies
Involve patients in decisions about their own health
Participate in case presentations/reviews and discussion within the team
Develop/research learning resources for health care providers
Present on various aspects of social work to TC FHT staff and physicians to assist them in their practice
Act as field instructor for students and coach/mentor other members of the health care team
Maintain current discipline specific standards of practice
Maintain awareness & understanding of current Mental Health legislation
Referrals/Collaboration
Collaborate with members of an interdisciplinary health team to plan, implement, coordinate and evaluate patient care in consultation with patients and their families
Liaise and consult with other agencies/programs/services as required
Provide communication to hospital and community staff and other inter-professional team members, as required, to ensure smooth transition for care between care sites
Administration/Reporting
Collect accurate and comprehensive patient-related information and document on clinical management system
Complete documentation required to facilitate care for the patient (reports, referrals, forms, etc)
Keep monthly statistics for the Ministry of Health and Long-Term Care (MOHLTC)
Maintain an organized appointment schedule
Monitor and manage referrals from the team and assist with waitlist mgt
Participate in program planning and evaluation within the team or externally as appropriate
Note
: On occasion, the social worker will provide support for homebound patients in conjunction with a physician assistant and an embedded Home & Community Care Support Coordinator for patients of TC FHT. In this capacity the social worker provides short term counselling, case management, advocacy and mental health consultations. The social worker builds supportive relationships with these patients and their caregivers. Some interactions will be in the person's home environment or in the context of accessing care.
Skills, Knowledge Requirements
Strong verbal and written communication skills and the ability to adjust language and terminology to suit the individual patient or patient group (including cultural sensitivity)
Excellent clinical skills
Autonomous and able to function with minimal supervision
Ability to work effectively with all members of the health care team
Ability to work in an inter-professional team environment
Proven exceptional interpersonal skills
Ability to prioritize, manage time effectively and be flexible in a very active work environment
Strong knowledge of community resources in GTA
Ability to remove obstacles standing in the way of adequate healthcare
Special Skills: Experience in providing trauma therapy an asset
Computer Skills
Familiarity with a Mac OS environment
Experience with Telus Practice Solutions (eMR) is an asset
Language Skills
English required; Second language not required but would be an asset
Education and Specific Job Requirements
Minimum Education: Masters Degree in Social Work
Registration and in good standing with the Ontario College of Social Workers and Social Service Workers
Working knowledge of the Mental Health Act, Healthcare Consent Act, Substitute Decisions Act and community mental health resources
Knowledge and skills in a variety of treatment modalies, including Cognitive Behavioural Therapy, Dialectical Behaviour Therapy, Mindfulness, Acceptance and Commitment Therapy, and trauma therapies.
Relevant Experience
A minimum of 3 years of experience working in mental health
Clinical experience working collaboratively with a community-based agency preferable
Teaching and experience facilitating treatment groups an asset
Reporting Structure
The Social Worker reports to the Lead Social Worker.
