We are looking for a
Term, Part Time Social Worker
to join our
Halifax
team!
Reporting to the Manager of Social Work, the Social Worker functions as a member of the inter-professional health care team while providing support, information, counseling and assistance to residents and families as they adjust to the communal living environment. The Social Worker is responsible the bed utilization function for their caseload at that location,for problem solving, as well as acting as an advocate on behalf of residents as well their family/friend caregivers.
Qualifications:
Bachelor's degree in Social Work.
Master's degree in Social Work considered an asset
Registered with the Nova Scotia College of Social Workers or enrolled as a Social Work Candidate with the NSCSW
Education and Clinical experience in Mental Health is required
Experience in dementia care, geriatrics, /or long-term care, palliative care preferred
Two to three years of relevant work experience
Strong interpersonal, verbal and written communication skills
Knowledge or experience with a person-centered model of care
Must have own vehicle or access to reliable transportation
Proficient in Microsoft Office (Outlook, Word, Excel, etc.)

Responsibilities:
Facilitates admissions process for new applicants (e.g., communicates with residents and/or family, orientates residents)
Participates in bed utilization (occupies vacancies and coordinates internal transfers)
Collaborates with other professionals to evaluate patients' mental health, medical or physical condition and to assess clients' needs
Provides one to one support to resident and/or families (e.g., discharge information, providing transitional counselling)
Supports and understands needs of people in transition
Serves as a resource for residents and their families advocating with internal members and community contacts
Counsels clients/residents
Provides conflict resolution
Chairs and/ or participates in care conferences
Maintains appropriate documentation on electronic medical record
Completes administrative duties as required
Travel between Northwood locations as necessary
