SOCIAL WORKER

Permanent, Full Time

Reporting to the Administrator, the Social Worker works in collaboration with an interdisciplinary team to advocate for and provide support and services to residents, families and staff.

RESPONSIBILITIES:

Works closely with Nursing Staff, the Leadership Team and other members of the interdisciplinary team. Develops a psychosocial assessment for each resident. Identifies and implements action plans to resolve complaints and concerns. Manages the admission process. Plans and leads care conferences. Liaison and secretary to Family Council. Facilitates partnership with families, staff and other agencies. Advocates and provides resources to residents, families and staff. Provides end of life support to residents, families and staff. Assists with financial concerns and works with Administration when needed.

QUALIFICATIONS:

Bachelor's degree in Social Work Registered under the Newfoundland & Labrador Association of Social Workers*
3 -5 years' experience in a long-term care setting preferred Must have a good understanding of Guardianship and Trusteeship. Superb organizational, time management and multi- tasking skills Knowledge of pertinent legislation, including the Adult Protection and the Advance Health Care Directives Acts Excellent oral and written communication skills are essential Resident advocate - focused attitude, with high emphasis on building and maintaining relationships with residents, families and staff
