Job Description

Klahoose First Nation (KFN) Health is looking for an experienced Registered Social Worker (RSW) to join our multidisciplinary team. The team provides a comprehensive range of services working within the context of a culturally safe and appropriate, holistic, client-centric focus. The RSW will work as part of the multi-disciplinary team at the Health Centre and will report to the Health Director.

Ideally, services are offered in person in the community of Squirrel Cove, on Cortes Island, however, for the right candidate, a hybrid model is possible. Cultural Safety is a priority of the Health Team as is direct client care coordination. The RSW will offer direct services for Klahoose FN community members living in Squirrel Cove as well as consultation, service navigation and care coordination, for Klahoose members living off reserve.

We are currently seeking a Registered Social Worker to play a pivotal role in providing comprehensive social services, assessments, interventions and referrals to individuals and families facing various challenges. Under the supervision of the Health Director, the duties and responsibilities of the Social Worker include, but are not limited to the following:

Conducts thorough and culturally sensitive assessments to identify clients' needs, strengths, and challenges across social, emotional, familial and economic domains Utilizes standardized assessment tools and interviews to gather pertinent information to develop tailored care plans. Collaborates closely with clients, families, and support networks to formulate individualized holistic care plans addressing identified needs and goals. Implements evidence-based interventions and strategies within the context of client's unique circumstances; Provides empathetic counseling, guidance and support to empower individuals and families facing social, emotional or economic difficulties. Advocates for client rights, access to services, and opportunities to enhance their well-being within the community. Klahoose First Nation SW liaises with community resource agencies, organizations to coordinate services and referrals for clients, ensuring access to comprehensive support systems; Fosters partnerships and maintains relationships with external stakeholders to facilitate the best outcomes for clients; Maintains accurate and detailed records of client interactions, progress notes, assessments and service plans in accordance with organizational and regulatory standards; Participates actively in multidisciplinary team meetings, case reviews and professional development activities and contributes to the shared knowledge base of the team. Assess and responds to crisis situations promptly, providing immediate support, intervention and appropriate referrals as necessary; Develops crisis intervention plans and strategies in collaboration with clients and the team. Stays updated on current trends, research and best practices in social work, integrating new knowledge and practice for continuous improvement.

The succesful candidate will possess the following qualifications and experience:



Bachelors or masters degree in social work. Current full registration in good standing with the BC College of Social Workers. Valid Driver's License and access to a reliable vehicle. Minimum three (3) years' experience providing social work services preferably in First Nation communities, in culturally sensitive diverse settings, specializing in mental health, child youth and family well-being, palliative care and rehabilitation related to mental health and substance abuse. Confidentiality is of the utmost importance. Demonstrated competence in mental health assessments, treatment methodologies, and intervention strategies. Strong interpersonal skills with the ability to communicate effectively and empathetically and the capacity to work independently and collaboratively within a multidisciplinary team. Knowledge of relevant laws, regulations, and ethical standards governing social work practices; Proficiency in documentation and record keeping practices; and commitment to ongoing professional development and upholding the highest ethical standards in social work practice. Ability to stand and sit and walk for extended periods of time and to lift a minimum of 40 pounds. Knowledge of MS Suite, Teams and other applications is imperative.
