Sinai Health is looking for an experienced Social Worker to support our Reactivation Care Centre team as part of our Allied Health- Social Work department.

About this team:

Our specialized 16 bed inpatient reactivation program is located on the beautiful and quaint UHN-RCC campus. The Sinai Health RCC program is designed for individuals who present with decreased mobility, independence and/or function as a result of an acute illness or surgery. We focus on patients with potential for functional improvement, providing tailored care plans and utilizing state-of-the-art tools to support their recovery journey. Depending on the reactivation goals patients typically experience an average length of stay of 30 to 60 days. You will collaborate with a high performing multi-disciplinary team including Registered Nurses, Registered Practical Nurses, Nurse Practitioner, OT, PT, SW, and SLP to provide high quality and safe patient care.

In this role you will:

Provide a range of social work services including assessment, crisis intervention, counselling; comprehensive discharge planning with individuals and families. Conduct psychosocial assessments and provide interventions based on a variety of issues such as aging and dementia, homelessness, addictions, child protection, grief counselling, mental health and domestic violence Link with community agencies to ensure coordination of appropriate services for patients. Job Requirements:

Mandatory


Master's Degree in Social Work Current registration with the Ontario College of Social Workers and Social Service Workers




Preferred


Experience with a range of care in a rehab setting Experience with consent and capacity




Skills and Knowledge


Demonstrated ability in individual and family counselling Knowledge of community supports and resources Demonstrated ability to work independently and in a team environment Excellent interpersonal, verbal and written communication skills Demonstrated interest in education and research Demonstrated satisfactory work performance and attendance record
