Overview

We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have a strong background in case management and experience working with children in a medical or hospital setting. This role involves providing support and resources to patients and their families, ensuring they receive the necessary care and assistance during challenging times.

Duties

Conduct assessments to identify the needs of patients and their families. Develop and implement individualized care plans based on patient evaluations. Provide counseling and support to children and families dealing with medical issues. Collaborate with healthcare professionals to coordinate patient care and services. Maintain accurate records of patient interactions, data collection, and case management activities. Research available resources and programs to assist patients in accessing necessary services. Educate families about medical terminology, treatment options, and available support systems. Advocate for patients' rights within the healthcare system.

Requirements

Bachelor's degree in Social Work or related field; Master's degree preferred. Experience working with children, particularly in pediatrics or hospital settings. Strong knowledge of medical terminology and case management practices. Excellent communication skills, both verbal and written. Ability to conduct thorough research to identify resources for patients. Proficient in data collection methods related to patient care. Compassionate approach with a strong desire to help others navigate complex situations. Certification or licensure as required by state regulations.

Job Type: Part-time

Pay: $85.00-$95.00 per hour

Expected hours: 10 per week

Schedule:

8 hour shift Monday to Friday Weekends as needed
Work Location: In person
