Overview




This position is responsible for the provision of clinical social work services to hospital patients and families and providing excellent patient care within an interdisciplinary team setting. Responsibilities include: providing comprehensive assessmentand treatment planning, educational and supportive counseling, resource identification, referral and linking to community services, and collaborating and consulting with the hospital based inter-professional team as well as community-based service providers. The primary area of coverage for this permanent role is the Emergency Department & Maternal Newborn Care, Hours of work may include 8, 10 and 12 hour shifts, days, evenings and weekends.


Education




Must be a member of the College of Registered Social Workers and Social Service Workers, or be a candidate for certification Master of Social Work from an accredited school of social work, or equivalent degree

Qualifications




Demonstrated ability to foster professional relationships with internal and external partners including social work and other health professionals, students, patients and families, colleagues and community agencies Ability to participate in professional, program and hospital development, planning, implementation and evaluation of service Strong written and verbal interpersonal and communication skills required Experience in Maternal/Newborn/Pediatrics, Mental Health & Substance Use, Geriatrics, & Chronic Disease preferred Demonstrated ability to prioritize and work effectively in a fast-paced environment Demonstrated patient & caregiver centered philosophy of care provision Demonstrated ability to welcome change and manage it innovatively Good work and attendance record required All employees of Michael Garron Hospital (MGH), a division of Toronto East Health Network (TEHN) [formerly Toronto East General Hospital (TEGH)] agree to work within the legislated practices of the Occupational Health and Safety Act of Ontario.
