As a Social Worker (bachelor's Level) at Foundry Penticton, you will provide time-limited, client-centred case management, system navigation, and supportive interventions to youth and their families. The role focuses on reducing barriers, promoting stabilization, and supporting young through key life transitions by connecting them with appropriate resources and services.
Working within a multidisciplinary team, you will support youth experiencing social, emotional, mental health, substance use, and life-transition challenges. This position emphasizes outreach, care coordination, crisis response as needed, advocacy, and culturally safe, trauma-informed practice within the Foundry integrated service model.
Qualifications and Experience
Bachelor's degree in social work (BSW) from an accredited institution
Registration (or eligibility for registration) with the British Columbia College of Social Workers (BCCSW)
Experience supporting youth aged 12-24 and families in community-based or integrated service settings
Experience providing time-limited case management, care coordination, and practical support
Knowledge of community resources, government benefits, and social service systems
Experience working with youth and families navigating mental health, substance use, housing, education, employment, and identity-related transitions
Strong documentation, communication, and collaboration skills
Required
Demonstrated cultural humility and understanding of diverse socio-cultural backgrounds
Familiarity with trauma-informed practice, harm reduction, and strength-based approaches
Valid First Aid Certificate (with CPR)
Successful completion of:
Criminal record check
Personal and professional reference checks
Valid BC Driver's License, reliable vehicle, and clean driver's abstract
Adherence to organizational policies, confidentiality agreements, and Code of Ethics
Flexibility to work evenings and weekends as required

Strong understanding of community-based social work practice and integrated youth services
Knowledge of mental health, substance use, social determinants of health, and family systems
Ability to provide crisis intervention and stabilization support when needed
Experience with system navigation and advocacy, including health insurance navigation
Ability to support youth with gender affirmation-related social transitions in an affirming and inclusive manner
Strong interpersonal, engagement, and de-escalation skills
Ability to work collaboratively within a multidisciplinary team and with external partners
Adaptability in fast-paced, evolving service environments
Strong written and verbal communication skills
Key Responsibilities
Case Management & Care Coordination
Provide time-limited case management to youth, including assessment of social needs, goal setting, and coordination of support
Supporting young in accessing community resources related to housing, income assistance, education, employment, health care, and social support
Assist clients with completing required documentation and applications, including PWD forms and related benefits
System Navigation & Advocacy
Support youth and families in navigating complex systems, including health insurance and medical coverage, income support, and community services
Advocate on behalf of clients with service providers, schools, health systems, and government agencies as appropriate
Support youth during key transitions, including aging out of services or moving between systems
Youth & Family Support
Provide practical, strengths-based support to youth and families to enhance stability and connection
Offer family support and education to strengthen understanding, communication, and engagement
Support youth with the social aspects of gender affirmation and identity-related transitions, in alignment with client goals and best practices
Outreach & Engagement
Conduct outreach to engage youth who may be disconnected from services or experiencing instability
Build trusting relationships to support engagement, retention, and stabilization
Participate in community-based outreach activities and collaborative initiatives
Crisis Response
Respond to crisis situations as needed, providing immediate support, de-escalation, and connection to appropriate services
Work collaboratively with internal and external partners to support client safety and continuity of care
Documentation & Team Collaboration
Maintain accurate, timely, and confidential client records in accordance with organizational standards
Participate in team meetings, case conferences, and service planning
Contribute to program development, service improvement, and continuous quality improvement initiatives
