As a recognized leader in end-of-life care, Grey Bruce Hospice-Chapman House provides specialized quality care, comfort and support in a 24/7, 9-bed Hospice Residence.
Grey Bruce Hospice is seeking a full-time (37.5 hours per week)
Social Worker
to join our team. Reporting to the Hospice Director, the Social Worker is responsible for providing system navigation and person-centered palliative care and grief and bereavement support for residents and their families. Working closely with our highly skilled team of care professionals, the successful candidate will provide direct client care to residents and their families to ensure their psychological, emotional, physical and spiritual care needs are addressed in a professional and compassionate manner. The Social Worker will identify and secure appropriate resources to support the resident's care plan by generating referrals to spiritual care, grief and bereavement support, complimentary therapies, volunteer support and/or other community providers. As a strong advocate for residents and their families, the Social Worker will actively review and evaluate care plan processes and outcomes to ensure the needs of clients and families are identified and adjusted according to changing needs through the end of life. This position is suited for someone with a keen interest in working with community partners and leading projects to expand the diversity of our client base by providing outreach to different patient populations.
Education and Qualifications:
Master of Social Work/Honours Bachelor of Social Work from a recognized University.
3-5 years of clinical practice experience/equivalent education and experience may be considered.
Current membership in good standing with the OCSWSSW.
Highly skilled in evidence-based methods and practices of social work (case management, system navigation, financial/advanced care planning and advocacy) in a hospice setting.
Willingness to support volunteers, providing guidance and training as needed.
Well-developed skills in creating, implementing, facilitating and evaluating groups.
Excellent interpersonal, critical thinking and problem-solving skills is a must.
Experience working effectively with diverse populations and equity focused initiatives.
Demonstrated ability to work independently and within an inter-professional team.
Compliance of resident performance indicators as stated in agreement with funders (OHT, M-SAA).
Efficient computer skills, and experience with electronic medical records/documentation
