The Allied Health Services (AHS) team, under Professional Practice consists of Regulated Healthcare Professional including: Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, Social Work, Registered Dietitians, Respiratory Therapy, as well as unregulated Rehabilitation Assistants. Each discipline is focused on driving evidence informed practice to promote high quality patient and family centered care. The team has a professional practice leader (PPL) model for each regulated allied health discipline which promotes and enhances the interprofessional team. Each discipline has their own PPL with dedicated time to enhance their communities of practice, represent their respective disciplines at committees, create policies & procedures and lead corporate level initiatives.

We are currently seeking a temporary full-time Social Worker to support our interdisciplinary team in our Emergency Department (ED), iCAN (Interprofessional Collaboration Assessment and Navigation). This initiative focuses on supporting the needs of high risk ED patients aged 65+, that may be able to transition back to the community through the use of a team based consultative services. The individuals would be expected to work in a team environment with a Geriatric Emergency Management (GEM) nurse, Occupational Therapist and Home and Community Care staff while consulting with the ED care team (physicians & primary nurse).

Education:

  • Graduate of an accredited School of Social Work with a Masters of Social Work- Required
  • Current registration with the Ontario College of Certified Social Workers/Social Services Workers -Required
  • Sound knowledge of related legislation as applicable to serviced population- Required
Experience:
  • Recent Social Work experience involving the Emergency Department and/or acute care settings - Required.
  • A strong theoretical and practical knowledge of social worker role and scope in promoting independence and assistance in acute care/emergency setting- Preferred.
  • Strong knowledge of community services, supports and experience related to facilitating family meetings and discharge planning- Required.
  • Experience with quality improvement initiatives or projects an asset.

  • Ability to function co-operatively as part of an interprofessional assessment team
  • Possess comprehensive assessment, treatment, and documentation skills related to biopsychosocial components of well-being
  • Possess excellent interpersonal and communication skills
  • Demonstrate sound clinical judgment, flexibility and initiative
  • Ability to communicate effectively with patients, families, caregivers, and other professionals
  • Superior organizational and time management skills are essential to management of the caseload
Other:
  • Ability to learn about, from, and with other members of the patient care team to foster a strong interprofessional model of care.
  • Demonstrate an ability to appreciate the importance of, and be open to new concepts and the ability to trial new approaches.
  • Ability to provide direction and synthesize information from other disciplines to create a comprehensive care plan and present recommendations to the most responsible physician.
Unless otherwise stated applicants must have a minimum educational requirement of Grade 12 or equivalent and must demonstrate proven workplace excellence through commitment to strong job performance and attendance.
