Position: Aurora-Newmarket Family Health Team (ANFHT) Social Worker Qualifications: Master of Social Work degree and experience in the mental health field, ideally both adults and seniors JOB SUMMARY: The Social Worker will play an integral role in the care of the patients of the Aurora-Newmarket Family Health Team by assisting the ANFHT in achieving its vision and mission. The role of the Social Worker in the Family Health Team requires a thorough knowledge of primary care within the scope of the MSW class and the ability to function effectively in a team environment with physicians, other integrated health professionals and the administrative team. The role also requires a high level of interpersonal skills to deal with patients and their families in a holistic compassionate fashion. Education and Qualifications: \xc2\xb7Masters of Social Work from a recognized University \xc2\xb7Current membership and in good standing with the Ontario College of Social Workers and Social Service Workers of Ontario \xc2\xb7Working knowledge of the Mental Health Act, Healthcare Consent Act, Substitute Decisions Act, other relevant legislation as well as community mental health and addictions resources \xc2\xb7Experience working as a Social Worker, MSW working with patients and families providing counselling \xc2\xb7Demonstrated knowledge and experience with a variety of crisis assessment and intervention techniques and support strategies for mental health and/or addictions \xc2\xb7Efficient computer skills, working knowledge of electronic medical records \xc2\xb7Experience in a multidisciplinary setting will be considered an asset Roles & Responsibilities: Assessment \xc2\xb7 Receive referrals from physicians and integrated health professionals (IHP) relating to patients with possible issues pertaining to mental health \xc2\xb7 Conduct preliminary sessions where appropriate and determine next steps \xc2\xb7 Detailed assessment of mental health issues relating to individual and family well-being \xc2\xb7 Write up initial findings and consult with ANFHT physician(s) to determine recommended plan of treatment \xc2\xb7 Field calls from persons in distress and direct appropriately; follow up to determine outcome Treatment/Management \xc2\xb7 Provide counselling, education and support to patient, family and other care givers \xc2\xb7 Provision of treatment through individual, couple, family and group counseling and psychotherapy to achieve optimum social and psycho-social functioning \xc2\xb7 Case management, including links to community services \xc2\xb7 Specifically for seniors, provide counseling for specific issues including adjustment to disability/illness, palliative care, intergenerational/family conflict, abuse, social isolation Education/Advocacy \xc2\xb7Participate with other team members in the provision of group programs and learning \xc2\xb7Identify and support access to community, volunteer and professional services and programs appropriate to the needs of the patient \xc2\xb7Provide patient education and individual and group sessions to patients in the understanding of particular health issues \xc2\xb7Assistance with system navigation and coordination \xc2\xb7Advocacy to access resources and supports Referrals/Collaboration \xc2\xb7Collaboration with other team members to optimize patient health \xc2\xb7Provide communication to hospital and community staff and other interdisciplinary team members to ensure smooth transition for care \xc2\xb7Referrals to community services, as appropriate \xc2\xb7Community outreach and linkages General Accountabilities: \xc2\xb7Respect \xc2\xb7Support and understanding of other team members Communications Skills:
