HOURS OF WORK:



The stated hours of work are approximate as schedules are subject to change with the operational needs of the Health Centre. Changes made to hours of work will follow the terms of the applicable collective agreement, company policy and/or legislation.


WHAT YOU'LL DO:



Social work provides service to patients and their families focusing primarily on the psychological and the emotional well-being of individuals as they adjust to the changes in their health. Social Work advocates on behalf of patients and families as they move through the health care system. Responsibilities include: biopsychosocial assessment, problem formulation, assisting in the development, implementation and ongoing monitoring of patient treatment plans, crisis intervention, and knowledge of community resources. In addition, the social worker provides information, consultation and education to patients, families, staff and community agencies on matters relating to their specialty. The Social Worker practices in accordance with the OCCSWSSW Professional Standards.


WHAT YOU'LL BRING:



Master's Degree in Social Work required 3 years' experience in medical/surgical social work in an acute care hospital setting preferred Member of the Ontario College of Certified Social Workers and Social Service Workers (OCCSWSSW) in good standing Membership in the Ontario Association of Social Workers preferred

Demonstrated expertise in all areas of clinical practice, (i.e. individuals, families and groups) Demonstrated expertise in a broad range of knowledge i.e. bio-psycho-social dynamic of illness and disease Commitment to providing services for patients and families, which address health from a holistic perspective (including physical, emotional, social, economic, spiritual and environmental influences on health) Excellent written, verbal and interpersonal skills Proficient use of computer applications (eg Word, Outlook) Ability to prioritize and adapt to unpredictable and changing situations Ability to work independently and also function as an effective member of the Interprofessional team Demonstrated experience in crisis intervention skills Ability to advocate on behalf of patients, families and/or the health care system Ability to assess and treat adults and families using an integrated model of practice utilizing hospital and community resources and the ability to work with these systems in the development of treatment and discharge planning Flexibility in presentation and use of self Knowledge of appropriate community resources Advocacy/brokering skills
