Journey Home Hospice, a program of the Saint Elizabeth Foundation, provides exceptional end-of-life care in a residential hospice setting. Our Social Workers bring person-centered care, build rapport and trust with patients, and enhance their quality of life. You will join a collaborative and supportive care team, applying your skills and heart to meet the physical, cognitive, social, emotional, and spiritual needs of patients and families.
Responsibilities:
Conduct comprehensive psychosocial assessments
Develop individualized end-of-life care plans to meet patients' psychosocial needs
Provide counselling, guidance, and advocacy for patients and families
Support patients and families in navigating community resources and services
Collaborate with interdisciplinary hospice teams to ensure holistic care
Participate in outreach activities, including community partnerships and family support programs
Liaising and assisting the Manager, Volunteer Services, as required
Qualifications:
Master of Social Work (MSW) from an accredited program
Member in good standing with the Ontario College of Social Workers and Social Service Workers
Recent experience in a healthcare setting, preferably palliative or hospice care
Strong interpersonal, organizational, and communication skills
Reliable, flexible, and self-directed professional, able to work collaboratively with a high-performance interdisciplinary team
