Position Description

The Hospital for Sick Children is a leading tertiary pediatric center providing medical and mental health care to the most complex children and youth and their families. A dynamic clinical, education and research environment, SickKids is committed to excellence in integrated inter-professional care. It is a center of excellence in the delivery of innovative health care to the paediatric population.
Reporting to the Clinical Director, Social Work, this position will provide social work services to both patients and families supported by the Sickle Cell, Thalassemia and Haematology Program in the Division of Haematology/Oncology at the Hospital for Sick Children. Through in-depth psychosocial assessments, the Social Worker identifies child and family strengths, problem-solving abilities, interactions, attitudes, feelings and cultural issues in relation to the child's/youth's illness as well as identify support and resources available to the family.
Here's What You'll Get to Do:
Develop and implement a plan of intervention which addresses trauma, coping, adaptation, treatment adherence and quality of life. This is achieved through a variety of modalities including using a trauma informed, culturally sensitive approach to counseling, consultation, case management, crisis intervention, group work, community outreach and advocacy
Support families in their adjustment to life changing acute and chronic diagnosis
Provide adjustment and supportive counselling
Collaborate with the interdisciplinary health care teams in both in-patient and out-patient ambulatory settings
Integrate research into practice
Provide psychosocial education to other members of the team in terms of social work interventions that benefit patients and families
Provide adjustment counselling around issues of depression/anxiety, immigration, acculturation
Support families and patients to improve coping and adjustment
Support families coping with illness by assessing resource issues secure resources related to the social determinants of health
Work collaboratively with a dynamic team in a fast paced ever changing environment
Potential for program development
Assume responsibility for all aspects of practice including issues of productivity and utilization
This is a new position joining a team of social workers and a unique psychosocial program in the Division. Along with working with individuals and families, there will be opportunities for program development and group work.
Here's What You'll Need:
Master's degree in Social Work, preferably with Paediatric hospital experience
Registration with the Ontario College of Social Workers and Social Service Workers
At least 3 years clinical experience of working with children and their families with complex medical needs within a hospital setting
Experience with child protection and/or trauma assessment and treatment
Training and experience in delivering psychotherapy is an asset
Well-developed family support skills
Knowledge of the network of community services and resources
Understanding and commitment to EDI informed practices
Understanding of health care delivery systems and the practice of social work in a secondary setting
Ability to collaborate effectively as a member of the inter-professional health care team and to take initiative and leadership when appropriate
Excellent interpersonal, communication, organizational, time management skills and attention to detail
Demonstrated critical thinking and problem-solving skills
Demonstrated commitment to practice in a manner that is consistent with the philosophy of continuous quality improvement and evidence-based practice
Proficiency in the use of electronic platforms such as Microsoft Office, hospital documentation systems and other relevant technology
Demonstrates commitment and actions in advancing equity, diversity and inclusion objectives
