Job Description

The Social Worker will work with the inter-professional team to provide services as required to patients attending for care following a stroke, ABI and SCI. The services provide range from psycho-social assessment and treatment; crisis intervention; advocacy; supportive counseling; consultation; supporting transitions and community re-integration; patient/family/staff education; and liaising with community resources.
Qualifications
1. Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker
2. Master's degree in Social Work from an accredited School of Social Work
3. Excellent knowledge of community support and placement services
4. Demonstrated knowledge and 2 recent years therapeutic experience within the last 5 years in the field of neurological rehabilitation.
5. Knowledge of Healthcare Consent Act, Substitute Decisions Act, Child and Family Services Act
6. Excellent crisis intervention and communication skills
7. Proven ability to work as a member of interdisciplinary healthcare teams, working both independently and collaboratively in a variety of settings
8. Proven ability to organize and prioritize patient caseload across several programs
9. Demonstrated skills in screening, assessment, counseling, case management and advocacy skills
10.Demonstrated interpersonal, communication, teamwork, consultation and teaching skills
11. Demonstrated commitment to ongoing learning
12.Demonstrated knowledge of the electronic charting system

Skills Required
