The Social Worker will conduct psychosocial assessments & develop treatment plans that include counseling, education and family intervention as well as securing the necessary community resources for patients and/or their families. The
Social Worker will also be responsible for facilitating & supporting the development of related cardiovascular patient program and professional initiatives and will perform other duties as may be assigned.
Qualifications
1. Master\'s Degree in Clinical Social Work from an accredited University
2. Current registration and member in good standing with the Ontario College of Social Workers and Social Service
Workers
3. Minimum 3 years recent experience with adults and families providing individual and family therapy
4. Minimum of 3 years of critical care experience preferred
5. Previous experience in providing bereavement crisis and counseling
6. Knowledge of Healthcare Consent Act
7. Knowledge of community resources and referral process
8. Demonstrated skills in the performance of a therapeutic role including strong assessment, counseling, case
management, crisis intervention and advocacy skills
9. Proven ability to work in a fast-paced, complex environment and to organize, prioritize and problem-solve effectively
10. Strong interpersonal, communication, teamwork, consultation and teaching skills
11. Demonstrated ability to contribute to an Interprofressional model of care team utilizing Family centered care principles
12. Demonstrated commitment to ongoing learning
13. EPIC hospital information system experience preferred
