Health Services, Long Term Care

Status: 2 Regular Full Time Positions

Salary Range: $86,443 to $108,052 per annum plus comprehensive benefit package

Locations:

Position 1: Peel Manor \xe2\x80\x93 525 Main Street North, Brampton, ON

Position 2: Tall Pines \xe2\x80\x93 1001 Peter Robertson Blvd, Brampton, ON

Hours of work: 35 hours of work per week - core business hours are 8:30am-4:30pm; Monday-Friday, however some flexibility may be required including being a part of a rotational schedule for manager weekend on duty and some evenings for events and meetings.

The Long-Term Care and Senior Services Divisions require staff to provide person-centered care to our residents and families following the Butterfly Model of Care. This will ensure high levels of emotional wellbeing and a better quality of life for those living in our homes. Staff are to have the emotional intelligence and ability to deliver person-centered care in an authentic way that is meaningful to the resident and their family.

Social Worker - Long Term Care

The role:
The Social Worker reports to the Administrator of the home and collaborates with other disciplines to manage admissions, transfers, discharge processes, resident interventions, liaising with community resources and provides emotional support to our families.

What you will do:

  • As a member of the interdisciplinary team, you will provide input into initial, annual and special resident assessment, care planning and evaluation to ensure services are planned and updated according to resident needs considering privacy, cultural diversity, intimacy, dignity and quality of life
  • Provide counselling and support to residents and their families related to admission, discharge planning, palliative care and bereavement.
  • Monitor the wait list and liaise with the Home Community Care Support Services resident admissions.
  • You will act as staff liaison and conduct tours for prospective residents and their families, develop and facilitate presentations for Residents\xe2\x80\x99 and Family Council meetings, chair ethics and palliative care committees and promote the centre within the community.
  • Coordinate in home events and supervise and evaluate social worker student placements.
  • You will complete resident documentation in accordance with the College of Social Workers and Social Service Workers standards, and maintain accurate CQI records, and dashboard entries, ensuring compliance with relevant Regional and Provincial acts, standards and accreditation standards. You will develop, implement and make revisions to relevant policies and procedures as required.
What the Role Requires:
  • A Bachelor of Social Work degree from a recognized university and membership in good standing with the Ontario College of Social Work and Social Service Workers.
  • Knowledgeable of the psychosocial needs of community seniors and the informal caregiver.
  • In-depth knowledge of available community services/programs to assist clients and their families to navigate the health and social service systems
  • Knowledge of relevant Regional and Provincial legislation including the Consent to Treatment Act, Substitute Decisions Act, MFIPPA, PHIPA, and policies and procedures.
  • Knowledge related to of the Consent and Capacity Board and Powers of Attorney
Skills/Abilities:
  • An effective leader with conflict resolution skills and strong problem-solving skills to be innovative and creative in developing strategies that successfully meet the needs of clients and caregivers
  • Demonstrated skills in assessment, facilitation and counselling, in long term care or a social service agency that serves the elderly
  • Strong communication skills both written and verbal and interpersonal skills to effectively work with clients, caregivers, staff, volunteers, and all internal and external resources.
  • Strong public speaking skills to conduct presentations to promote community support services.
  • Strong administrative skills including ability to formulating and administer the budget.
Nice to have:
  • Additional courses in gerontology, and experience with Alzheimer\xe2\x80\x99s disease and related dementia\xe2\x80\x99s an asset
