Permanent Full Time - FTE 1.0 (37.5 Hours per Week

)

Community Care, City of Kawartha Lakes (CCCKL) is seeking applicants for a Full Time Social Worker (Masters) to provide assessment, support, case management, counselling/therapy, and education to Community Health Centre (CHC) clients. The Social Worker participates as a member of the interdisciplinary team and assists in the ongoing development of primary care programs. This position works collaboratively with other team members to organize and optimize the care of CHC Clients, supports clients and families in a holistic, compassionate manner and helps navigate community supports as well as resources.



KEY ACCOUNTABILITIES





Support the CHC to provide assessment, support, and determine the best course of treatment for clients.


Provide assessment of mental health and psycho-social issues related to individual and family well-being.
Determine client's specific needs, goals, characteristics, problems, and stage of change through mutual investigation and exploration.
Conduct interviews to determine possible developmental delay.
Provide counselling services with particular regard to individual and family issues and decisions that impact the health of clients and families.
In conjunction with individuals and families, develops and implements meaningful and attainable care plans.
Provision of treatment through individual, couple, family and group counseling and psychotherapy to achieve optimum social and psycho-social functioning.
Case management, including links to community services and supports. Advocates on behalf of clients to ensure access to services that meet the client's needs.
Counselling for specific issues including adjustment to disability/illness, palliative care, intergenerational/ family conflict, abuse, parenting, and social isolation.
Provide crisis intervention as needed.
Attend case conferences
Educate and advocate on behalf of clients
Provide referrals and collaboration to ensure ongoing care of clients
Ensure support of CHC strategies, clinical best practices, and adherence with legislation.


KEY REQUIREMENTS AND COMPETENCIES





Masters in Social Work and experience working with families in a health setting, including children and youth
3 to 5 years' experience in a community setting, or an equivalent combination of education and experience.
Ability to establish and maintain rapport with clients
Demonstrated problem-solving and critical thinking skills
Excellent oral and written communication skills
Thorough knowledge of and proficiency on current assessment, psychotherapy, and psycho-social counselling techniques with individuals, families, couples, and groups.
Proven ability to work as part of an inter-professional team
Demonstrated ability to identify and meet personal learning needs in pursuit of ongoing professional development


WAGE





The wage range for this exciting opportunity is

$38.70-$42.38

. Selected candidates will be appropriately placed on the organizations wage grid based on skill, knowledge and experience.*
