Since 1949, TVCC has been working with children and youth to help them reach their full potential. Today, a wide range of services and supports are provided for young people with physical disabilities, communication disorders, developmental delays and autism spectrum disorders to support them in attaining the best available quality of life and health. In partnership with local and regional agencies, TVCC annually serves more than 11,000 children, youth and their families from birth to age 19 across Southwestern Ontario in over 700 locations.

We are currently recruiting to fill a permanent, full-time (37.5 hours per week) Social Worker in Family and Community Services. The designated office for this position will be London, meeting client needs throughout Southwestern Ontario. Regular travel will be required. The successful candidate will be required to work occasional evenings.

Responsibilities:



Meet with families in person, over the phone, and through virtual platforms to assess their needs in relation to their child's diagnosis and create a plan of support; Provide psychosocial support and guidance to families, children, and youth within a holistic and comprehensive framework using a variety of methods to help them to: adjust to a new diagnosis; explore stressors, coping strategies, and strengths; plan for transitions; advocate for their child's needs; and access resources; Offer services in a tiered model by creating universal supports, sharing information at workshops, leading group interventions and providing one-on-one intervention; Assist families to identify, plan and coordinate services for their child; Connect families to supports and services in their communities and help them navigate service systems; Educate and support families to enhance abilities to advocate positively within school and community environments; Build relationships within the community to develop a foundation for success for clients and their families; Help the family with practical considerations such as identifying financial supports, completing paperwork, solving issues such as transportation, or finding informal supports in their community; Work within a variety of our Specialty Clinics and programs supporting diagnosis-specific needs, including but not limited to Blind Low Vision Program, Cerebral Palsy Clinic, Cleft Lip and Palate & Clinic, Infant Hearing Program, Neuromuscular Clinic, Rett Syndrome Clinic, and Spina Bifida Clinic; Participate in planning services as part of a multi-disciplinary team including care providers, volunteers, staff from other agencies; Provide community and Centre-based intervention within the Life Needs Model of Service; Participate in service development and evaluation; Participate in and /or integrate and share current research/literature on disability, family and community development.

Qualifications:



Master's degree in Social Work (MSW); Current registration (or eligible for registration) as a Social Worker within the Ontario College of Social Workers and Social Service Workers (OCSWSSW); Knowledge of current models of service provision and clinical expertise relating to children and youth with disabilities and their families; Knowledge of children's services including rehabilitation, mental health, childcare and education; Previous clinical experience and skill with assessment, family system analysis, strengths-based interventions and individual brief counselling; Strengths in interpersonal relations, communication, and problem solving, with the ability to support a multidisciplinary team in stressful family and community work environments; Strong clinical telephone skills and have a deep belief in the positive impact that telephone exploration and intervention can have with families; Current knowledge of best practices and literature related to disability, family and community development and the Life Needs Model; Computer proficiency including Microsoft Office 365 package of software and video conferencing software (e.g. Teams); Current certification in First Aid and CPR; Valid Ontario driver's licence and access to a vehicle are required; Ability to provide services in French is considered an asset.

Salary range:

$36.32 - $45.40/hour
