The Leduc Beaumont Devon Primary Care Network is a joint venture between participating local family physicians and Alberta Health Services. Our mandate is to sustain and enhance the quality, coordination, and integration of healthcare services in Leduc and the surrounding area, and to improve the quality of life for the community of patients and physicians.
The Leduc Beaumont Devon Primary Care Network is seeking a Social Worker (SW) to join the multi-disciplinary healthcare team. The social worker will provide comprehensive social work services to referred patients within the Leduc Beaumont Devon Primary Care Network. The SW will assess, plan, implement, and evaluate social work care plans for eligible and appropriate patients. The SW will work in collaboration with all LBD PCN providers and member physicians, leveraging existing local and regional supports. The SW will work to their full scope of practice within the primary care setting.
Duties and Responsibilities:
Complete a thorough (bio)psychosocial assessment of referred patients/families and work with the patient/family to prioritize needs. Determine a relevant treatment plan and follow-up to ensure goals are implemented and barriers are addressed.
Assist patients and their families in accessing financial support, housing, employment opportunities, and other resources that can improve the patient's quality of life.
Support patients, their families, and caregivers in navigating health and social systems, and advocate for them within those systems.
Refer to other members of the Team-Based Care team when appropriate.
Act as a social work resource to other clinical team members and member physicians.
Develop, coordinate, and/or participate in presentations and groups to support patients, families, caregivers, staff, member physicians, and the public. Ensure accurate, objective, and timely clinical documentation in the appropriate medical record.
Requirements:
Bachelor's degree in social work, with current registration with the Alberta College of Social Workers.
Previous experience in a primary care setting; group facilitation would be an asset.
Understanding of Alberta privacy laws.
Demonstrate excellent written and verbal communication skills.
Salary and Hours of Work:
The salary range for this position is $38.56 to $49.20 per hour
Hours and days of work will be negotiated with the successful candidate.
