Reporting to the Director, Psychosocial & Adult Services and working in collaboration with the clinical Social Work Team. This position will span the Burnaby Key Worker program and the Foundry Burnaby Youth and Family Advisory Committees. Both programs are aligned in supporting young people and their families in understanding and navigating systems of care, promoting inclusion, and providing individual and group support around complex social issues.
The Key Worker program supports children at youth 0-19 diagnoses with FASD and other prenatal substance use exposure and their families, using a Family-Centred Practice model. Key Workers provide individualized, strengths-based intervention to promote the social-emotional development of children and youth, enhance family functioning and strengthen parent capacity to support their child. This role will provide individual support and facilitate groups.
Foundry Burnaby is currently in development. This role will support the monthly meetings of the Youth Advisory Committee and Family Advisory Committee. These groups will meet regularly to discuss issues relating to youth mental health in Burnaby to inform the development of Foundry Burnaby.
Responsibilities
Supports a caseload of families within the Key Worker program
Provides psycho-social assessment, creates the service plan, and reviews outcomes in a timely manner.
Works in collaboration with parent/caregivers to provide individualized support to children, youth and their families.
In collaboration with other social workers, facilitates groups for clients and their families with opportunities for parent and family networking.
Provides follow up support to clients as required.
Maintains appropriate documentation and client records in accordance with established policies and procedures and BCCSW standards.
Maintains workload statistics as required.
Contributes to program reporting.
Promotes and enhances inter-agency collaboration and communication in Burnaby by participating in inter-agency committees/meetings relating to well-being of young people in Burnaby.
Provides presentations to community organizations about Key Worker and Foundry Burnaby services.
Runs monthly meetings for Foundry Burnaby Youth and Family Advisory Committees and provides follow up support and communication to committee members between meetings.
May involved occasional evening and weekend work.
Performs other related duties as assigned.
Qualifications
Education, Training and Experience
Master's degree in Social Work from an approved school of Social Work
Minimum of 2 years of experience working with children, youth, and families with disabilities and/or mental health concerns.
Experience facilitating groups is preferred.
An equivalent combination of education and experience may be considered.
Knowledge, Skills and Abilities
Knowledge and understanding of social-emotional development, family-centred practice, and cultural humility and safety practices.
Proven collaboration, communication and consultation skills
Ability to communicate effectively, both verbally and in writing
Ability to work independently and as part of a team
Proven organization and time management skills are required
Ability to adapt to change and embrace new technology
Competency using a virtual platform, electronic client record system and word processing software
Working Conditions & Provisos
Must be registered with the BCCSW (regulatory body)
Valid driver's license with reliable use of car for work with business insurance
Physical ability to carry out the duties of the position
