The Primary Health Division is currently seeking a Social Worker III to work as a member of the interdisciplinary team working within the vulnerable population sector, providing services to individuals experiencing homelessness within the St. John\'s Metro region. Care is designed to provide patient-centered, evidence-based, compassionate, comprehensive, sustainable care to support individuals to obtain, and maintain, more permanent housing.The social worker will provide clinical social work services to individuals with complex health issues, which may also be impacted by mental health and addictions issues. As part of an interdisciplinary team, you will provide psychosocial assessment and therapeutic interventions including individual and group counseling, crisis intervention, support, advocacy, patient and family education and educations of other staff who may be within the circle of care. Other responsibilities will include conducting risk assessments, assisting clients in accessing required community services, helping clients recognize, develop and utilize support networks and initiating appropriate community referrals; and building life skills.The Social Worker III functions autonomously and uses independent judgment in developing, implementing, and analyzing treatment goals. You are knowledgeable about mental health and addictions diagnoses and issues, housing and homelessness, supports and services, healthy living and health promotion strategies, trauma informed care, poverty and homelessness, and mental health and addictions. You apply evidence informed practices to support clients experiencing these issues. Extensive knowledge of community resources and knowledge of systems available to support clients is essential. You will use a strengths-based approach and will be required to have flexibility in your hours of work and a willingness to be creative in motivating and inspiring clients. The social worker will facilitate and participate in team consultation and case conferencing, serve on committees and be involved in program planning and development as required. The Social Worker III will also be required to provide clinical leadership to other social work positions as the program expands and develops.The Social Worker III will be responsible to provide field instruction to social work students and offer other clinical education experience to students within Eastern Health. This position maintains accountability to the manager of the Collaborative Team Clinic and to the Professional Practice Consultant for Social Work for integrity and professional practice issues.Job Demands

Graduation with a Clinical or Integrated Master\'s degree in Social Work from an accredited program with the MSW field placement in a clinical practice is required.
Current and continuing registration with the Newfoundland and Labrador College of Social Workers (NLCSW) is required.
Minimum of five years of experience within the last seven working as a Social Worker in a health care setting or mental health and addictions program is required.
Experience with individual, family and group therapy is required.
Experience in brief therapy modalities is an asset.
Knowledge of intensive case management and coordination of services.
ASIST training is an asset.
Knowledge of relevant legislation, including but not limited to, Mental Health Care and Treatment Act, Adult Protection Act, Advanced Health Care Directives Act, Medical Assistance in Dying, and the Children, Youth and Families Act.
Knowledge of the impacts of poverty and homelessness on physical and mental well-being.
Knowledge of the Recovery Model.
Knowledge of mental health, addictions, concurrent disorders, trauma and associated treatment approaches.
Knowledge of community resources in the areas of mental health and addictions.
Knowledge of crisis interventions and supportive counseling.
Demonstrated strong clinical leadership skills.
Demonstrated ability to direct, supervise and mentor students.
Strong program development and evaluation skills.
Demonstrated proven analytical, critical thinking, problem solving, decision-making and organizational and research skills.
Strong written and oral communication skills as well as the ability to deliver/facilitate effective presentations.
