Your Opportunity:
Social Worker III - Prehabilitation Individualized Services Team: Make your impact where it matters--help transform rehabilitation services for pre-surgical clients with complex needs as a Social Worker III- Advanced Clinical Practice. This advanced clinical role supports adults in the community who are awaiting surgery and benefit from individualized rehabilitation to optimize outcomes. As part of the Prehabilitation Individualized Services Team, you'll deliver holistic, patient-centered care through biopsychosocial assessments, therapeutic interventions, and collaborative care planning. Working within an interprofessional team (PT, OT, Dietitian, Nurse, Psychologist etc.), you'll provide direct and indirect services in both in-person and virtual formats. Responsibilities include maintaining clinical documentation, mentoring students and staff, and offering rehabilitation consultation to team members and community partners. Key interventions include crisis response, brief counseling, safety planning, psychosocial support, and education around advance care planning, substitute decision-making, and local system navigation. You'll also foster partnerships across the province to enhance continuity of care and rehabilitation readiness. 15- 20% of this advanced clinical practice role can be used for team-based leadership, including interdisciplinary service evaluation for team and clinicians, offering consultation, mentoring staff and students, project work, and quality improvement. You'll also contribute to Connect Care optimization, clinical education, and program development, while promoting trauma-informed, culturally responsive care. As a Social Worker III, you will perform either advanced level assessment and treatment or act as a clinical lead. You will work with or within teams to provide health care services for patients with complex and multidimensional medical, psychosocial, environmental and instrumental needs. You will provide advanced clinical services, consultation to the interprofessional team and patients/families and supervision of students and team members as assigned. You may also act as a clinical lead providing leadership in the planning, development, and delivery of services in a specific area, as well as staff development, quality improvement and evaluation or research activities as identified by the needs of the service area(s).

Required Qualifications:

Master's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

A minimum of 3-5 years' experience as a rehabilitation clinician in community/ ambulatory settings is required. Candidates must be authorized to perform restricted psychosocial interventions AND hold a master's degree in social work. Strong clinical expertise in adult rehabilitation is essential, along with training in CBT, DBT, group psychotherapy, family therapy, or other recognized modalities. The ideal candidate thrives in dynamic environments, demonstrating adaptability, critical thinking, and sound decision-making both independently and within a team. Excellent communication, assessment, counseling, and organizational skills are required, as is the ability to build therapeutic and collegial relationships and manage sensitive situations. Responsibilities include maintaining clinical records, offering consultation, mentoring staff and students, and educating clients on advance care planning, substitute decision-making, and local system navigation. Experience with Connect Care and digital tools (EMRs, virtual platforms, email, word processing) is expected. The role involves moderate physical activity (lifting 20-50 lbs.) and extended seated work with technology. High cognitive load is expected due to point-of-care charting and mixed (virtual and in-person) service formats. Minimum two years of formal or informal leadership experience is required.

Preferred Qualifications:

Training in HealthChange Methodology(TM) and experience in virtual and group service delivery is considered an asset.
