We are seeking an experienced individual to join our team in a Social Work III - Team Lead position. Reporting to the Social Work Manager, Allied Health, the Team Lead will work across the centralized SW team to support, develop and enhance inpatient and ambulatory social work services, adhering to evidence-based practices, professional standards and applicable legislation, policy, and guidelines. This SWIII role provides an opportunity for advanced clinical practice as you will be responsible for the day to day leadership and coordination of staff and service delivery within assigned areas, and support the integration of social work into interdisciplinary care teams; you may also provide direct clinical services. The Team Lead will participate in orientation, coaching, and mentoring on complex psychosocial issues impacting patient care, safe discharge planning, monitoring progress and evaluation, and supporting best practice of core clinical competencies. As a Team Lead, you will be a critical thinker and apply a systemic perspective to advocate and lead in an empathetic, trauma informed and culturally competent manner to staff, ensuring delivery of high quality patient and centered family care.
Description:
As a Social Worker III, you will perform either advanced level assessment and treatment or act as a clinical lead. You will work with or within teams to provide health care services for patients with complex and multidimensional medical, psychosocial, environmental and instrumental needs. You will provide advanced clinical services, consultation to the interprofessional team and patients/families and supervision of students and team members as assigned. You may also act as a clinical lead providing leadership in the planning, development, and delivery of services in a specific area, as well as staff development, quality improvement and evaluation or research activities as identified by the needs of the service area(s).
