The Urgent Care Clinic provides timely, low-barrier access to specialized mental health care. An interdisciplinary team delivers assessment, brief intervention, and stabilization supports for individuals in crisis. The clinic aims to reduce pressure on emergency departments and enhance continuity of care across the system. Services are delivered in close collaboration with partners, including police, paramedics, and local emergency departments.
Duties:
Apply best practices and innovative methods for client-centered assessments, treatment, and psycho-social support.
Collaborate with clients to create care and crisis plans.
Assist clients with accessing services for addiction, housing, employment, relationships, daily living, leisure, and transportation.
Work effectively with team members and community partners, including hospitals and emergency services.
Demonstrate strong knowledge of social work theories and practices for individual and family interventions.
Participate in the design, implementation, and evaluation of services.
Maintain accurate documentation in the Electronic Health Record (EHR).
Ensure documentation meets hospital and College standards and timelines.
Advocate for client services and opportunities with community partners.
Stay current on hospital policies, procedures, and legislation, education, and networking.
Keep up with processional standards through literature, education and networking.
Participate in program, hospital and community committees, including education, advocacy, and system development.
Provide services both on site at the Urgent Care Clinic and off-site in community settings.
Supervise Social Work Students.
Follow staff and client safety practices and hospital policies.
Promote an environment aligned with anti-racism and harassment-free workplace policies.
Qualifications:
Master's degree in Social Work.
Current registration and member in good standing with the Ontario College of Social Workers & Social Service Workers.
Must have minimum of three (3) years of clinical Social Work experience working in a mental health and addiction setting.
Training and experience in crisis intervention, assessment, consultation and addictions in a community based setting.
Knowledge of MHA.
Excellent knowledge of community resources and referral processes.
Collaborative approach to working with community partners.
Strong conflict resolution abilities.
Excellent advocacy skills.
Proven presentation, training and group facilitation skills.
Demonstrated ability to work independently.
Effective team skills.
Able to work in a progressively automated environment.
English level A- is mandatory in oral expression, oral comprehension, reading comprehension and written expression. Bilingual (French/English) is an asset.
Salary Range:
44.67 to 57.90 per hour
