Your Opportunity:As a member of the Central Intake Team for Restorative Care and Rehabilitation Unit beds in Central Zone, you will be responsible for completing referral intakes, matching patients with the most appropriate program, addressing barriers to accessing a program, and supporting patients and families as they make decisions about their rehabilitation journey. The team processes referrals for Unit 35 (Rehabilitation Unit) at RDRHC, the Stroke & Geriatric Empowerment (SAGE) Unit in Two Hills, the Daysland Orthopedic and Active Rehabilitation (DOAR) program, the Restorative Care program in Lacombe, and the Early Supportive Discharge (ESD) stroke program in Camrose. The Central Intake Team consists of both Social Worker II positions, as well as a RN positions and an administrative support position. This team is located in the South Complex at Red Deer Regional Hospital Centre and onsite work is required. This is a relatively new service and part of the role includes implementing an evaluation plan. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
