This position is part of the centralized Rehabilitation and Allied Health Program and will be based in Grande Prairie, AB to serve local patients. The Renal Social Worker within Alberta Kidney Care-North provides psychosocial support to individuals with kidney disease, their families, and caregivers. They assist patients in navigating the complex healthcare system, managing the emotional and practical challenges of living with kidney disease, and accessing necessary resources. They work closely with other healthcare professionals to ensure a holistic approach to patient care. Key responsibilities include conducting comprehensive psychosocial assessments, adjustment counselling, resource navigation, advocacy, collaboration, education, documentation, and interdisciplinary teamwork. You must be highly motivated, confident, and enthusiastic individual with a positive attitude whose work aligns with AHS Values.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
