Your Opportunity:As a Social Worker-Discharge Coordinator, out of the Northern Lights Regional Health Centre, you will have the opportunity to work within a unique diverse environment and enjoy the benefits of a larger team while collaborating with other professionals. As part of a team, you will support clients within the hospital setting to facilitate access to community resources and enhance care through effective communication with the care team. You will support client transitions across the care spectrum of the departments. In addition to a competitive rate of pay, AHS currently has a Fort McMurray Northern Living Allowance in place to an annual maximum of $12,480. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.Description:As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
