Within Cancer Care Alberta (CCA), Supportive Care services are provided by a team of health care professionals dedicated to enhancing the quality of life of those affected by cancer in Alberta by providing support from the time of cancer diagnosis, through treatment and beyond. The professional team within CCA are leaders in cancer prevention, diagnosis, treatment, survivorship, and palliative care, all on a foundation of world-class research. The Social Work (SW) team at the Cross Cancer Institute needs a permanent part-time SWII to join the team. The SWII plays a crucial role in empowering patients with cancer to address the social determinants of health. You will provide bridging support and supportive counselling. If you are a patient-centered collaborator with social work experience and a passion for working in cancer care, this job is for you. Reporting to the Manager, the SWII is involved working to full scope of practice in referrals triage, screening, assessment, intervention, quality improvement initiatives, research & service evaluation. The successful applicant will support and consult with patients/families, colleagues, and internal/external agencies to provide coordinated interdisciplinary care, including resources and supportive counselling. This role supports inpatient units, but also outpatients as required. The SWII engages in continuous professional development & education. With the team, you will support orientation/mentorship of new staff/students. Effective time management & excellent written/verbal communication skills are essential in this role. You will be skilled at developing collaborative partnerships with community programs to help facilitate access to services for ongoing needs. Caseload for all positions within Psychosocial Oncology subject to change based on operational considerations. Some flexibility in hours of work over 3 days/week.

Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
