This position is part of the centralized Rehabilitation and Allied Health Program, responsible for providing social work services to the adult population across acute care, with a focus in the Neurosciences area. The candidate will be a highly motivated, confident, and dependable individual who collaborates with team members, patients and their families in a fast-paced environment ensuring trauma-informed, equitable care. The candidate will hold working knowledge of health care systems, SDoH, and community resources to support psychosocial interventions such as complex discharge planning, financial assessments, crisis intervention, decision making, advocacy, and referring to internal/external agencies. As a Social Worker II you are part of a team driven to provide quality patient-centered care though best practices and evidence informed approaches and in alignment with AHS Values.
Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
