Your Opportunity:We are looking for a personable, reliable, and skilled individual for a temporary full time (1.0 FTE) Social Worker II. This position is multi-site, working out of the Addiction and Mental Health Clinic in the Cardston Provincial Building and the Addiction and Mental Health Clinic in the Fort Macleod Health Centre, on a rotation. The Social Worker will perform social work-based assessments and create appropriate social work treatment plans for clients and their families, including supportive counselling and case management. The successful candidate will help connect the clients to community resources and provide the necessary support and advocacy to assist with housing, AISH, Social Services, and the like. As a regulated Social Worker, the successful person will maintain their own caseload of clients with complex needs. This position would be ideally suited to a social worker who has authorization to perform psychosocial interventions, or a willingness to work towards this. The successful person for this role will be a Registered Social Worker with a BSW, and have considerable experience in crisis counselling, conflict resolution and case management. To be successful in this role the Social Worker requires an in-depth knowledge of community resources as well as contacts within same. The role requires excellent interpersonal skills and the ability to function well as part of a small team as well as independently. This position reports to the Manager of the Rural Addiction and Mental Health Community Clinics.Description:As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
