Your Opportunity:The successful applicant will be a member of an inter-disciplinary team based out of Camrose Home Care and will be required to provide a range of social work services to patients and their families. These services may include: Psychosocial assessment. Resource-based interventions. Individual, group and family treatment. Risk screening/counselling. Case management and coordination. Transition or discharge planning. Prevention, promotion and public awareness activities. As this is a home care position, this does require the successful applicant to have a valid driver\'s license and operating vehicle. They will be required to travel to clients\' homes in the surrounding areas. Knowledge around seniors\' programs, government benefits and resources is considered an asset.Description:As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Required Qualifications:Completion of bachelor\'s degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW). Additional Required Qualifications:Requires current/up to date BLS certification. Minimum 3 years\' experience as a Social Worker.
