Your Opportunity:
We are actively seeking a full-time Social Worker II to work on the Concurrent Rehab Unit within the Claresholm Mental Health and Addiction Care Centre in the town of Claresholm. Claresholm is a vibrant rural community that is located approximately one and a half hours drive from Calgary and one hour drive from Lethbridge. This position will be responsible for facilitating and coordinating client discharge plans and delivering some group work services in conjunction with the program team. There are two separate Satellite Programs with 6 beds each within the Claresholm Care Facility. This position's responsibilities are in accordance with the mission, philosophy, objectives and policies to the Claresholm Centre for Mental Health and Addictions (CCMHA). Reporting to the Clinical Operations Manager, this role includes administrative, educational, clinical and liaison activities. Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW). Additional Required Qualifications:
Background in the mental health field with working knowledge of mental health disorders. Proven skills in communication, organization, and group therapy facilitation. Proficiency in Microsoft Office, internet, and email applications is essential. This position has transitioned to Recovery Alberta effective September 1, 2024. Experience in discharge planning within an Addictions and Mental Health setting, with knowledge of trauma-informed care, Collaborative Problem Solving, and community-based resources. Strong relationship-building skills with internal and external stakeholders, and the ability to support individuals with complex mental health and substance use needs. Completion of Non-Violent Crisis Intervention (NVCI) training and familiarity with Collaborative Problem Solving are considered assets.
