Your Opportunity:
Reporting to the Care Manager within Integrated Supportive and Facility Living (ISFL), the Occupational Therapist (OT) works collaboratively as an integral member of the interdisciplinary healthcare team within the full scope of their professional practice. This position provides clinical consultation to various Supportive Living sites to assess, implement, and evaluate client treatment plans relating to occupational therapy services. The OT develops and implements programs to optimize health and functional independence. The OT is responsible for ensuring that practice meets legislative requirements and the Alberta College of Occupational Therapists (ACOT). The OT is guided by their level of education, expertise, experience, demonstrated skill level and AHS policies and job description.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
