Your Opportunity:The Social Worker within the Endocrinology and Metabolism Program contributes primarily to Diabetes Centre Calgary, Calgary Diabetes in Pregnancy Clinics and the Endocrinology team. The successful applicant will work as part of a strong multidisciplinary team including Psychologists, Endocrinologists, Diabetes Educators and community partners. Responsibilities include assessing patients\' social support, housing, income support, connecting patients with resources as appropriate, supporting with illness adjustment and counselling. The Social Worker will consult on patients and make referrals to other team members for professional services as needed. The Social Worker\'s roles may overlap with other team members in care planning and requires a strong degree of collaboration and coordination.Description:As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:Completion of bachelor\'s degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW). Additional Required Qualifications:Minimum 2 years of clinical experience as a Registered Social Worker. Excellent written and verbal communication skills. Intermediate computer skills including proficiency with electronic medical records. Demonstrated skills in addressing psycho-social needs in chronic disease population. Demonstrated ability to work independently as well as part of a multidisciplinary team. Current BCLS required. Preferred Qualifications:Additional training/certification in counseling.
