Your Opportunity:

Located in N.E. Calgary, the Peter Lougheed Centre (PLC) is an acute care hospital offering both inpatient and outpatient services. The PLC has over 600 inpatient beds, including Medical, Surgical, Pediatrics, Women\'s Health and Critical Care. The PLC is the Vascular Centre for Southern Alberta, Southwestern Saskatchewan and Southeastern British Columbia. The Allied Health Department at the PLC is comprised of Social Work, Rehabilitation, and Spiritual Care staff. These staff work as vital members of the interdisciplinary team to provide patient and family-centered care throughout the hospital. Under general supervision from the Clinical Leader and Allied Health management, the successful candidate will provide Social Work services to the acute care units at the PLC. These services are primarily clinical in nature, and include psychosocial assessments, counseling, case management and discharge planning. This caseload may change based on operational needs. In addition, support to other patient care units will be required to manage daily workload across the Social Work team. This position requires the ability to work independently in a fast-paced environment and yet function as part of the interdisciplinary team. At all times the Social Worker ensures he/she is compliant with ACSW standards of practice.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor\'s degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW). Additional Required Qualifications:

Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification required. Excellent interpersonal and communication skills to interact with members of the interdisciplinary team, patients and families in a dynamic health care setting. Demonstrated problem solving, critical thinking and conflict resolution skills. Excellent organizational and time management skills to efficiently organize and prioritize workload to keep pace in a busy multi-disciplinary patient care. Demonstrates behaviours consistent with the values of AHS - Compassion, Accountability, Respect, Excellence, and Safety. Experience working with diverse communities, groups and/or organizations (i.e. ethno-cultural, equity-setting, racialized, faith-based, LGBTQ2S+, vulnerable, marginalized, persons with disabilities, and persons across all age groups, etc.). Preferred Qualifications:

Recent Social work experience within an acute care setting. Previous experience in medical social work is desired. Recent experience working with patients/families facing chronic illness preferred. Previous Connect Care Training an asset.
