Job Description

Bayshore HealthCare is seeking a compassionate and dedicated Social Worker to join our growing team in The Greater Hamilton & Burlington Area. This role offers the opportunity to support clients in home and community care settings, helping them navigate life's challenges and access essential resources.
Key Responsibilities

  • Conduct assessments and develop individualized care plans
  • Provide counseling, crisis intervention, and resource navigation
  • Collaborate with healthcare professionals, families, and community agencies
  • Advocate for clients' needs and support their independence and well-being
  • Maintain accurate documentation and comply with professional standards

Qualifications
Qualifications
  • Degree in Social Work
  • Registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • At least one (1) year experience in community health, mental health, or case management
  • Strong communication, organizational, and problem-solving skills
  • Minimum class G2 Driver's License and access to a vehicle for community visits
