Job Description

The Social Worker coordinates and facilitates psychosocial supports for patients and family members at the time of diagnosis, during treatment and during discharge planning. The Social Worker provides psychosocial screening and assessments patients and family methods of coping., providing crisis intervention, assist with adaptation to stresses of treatment and hospitalization, provides education and access to necessary resources, counsels and participates in discharge planning and provides bereavement counseling. Collaboration with the interdisciplinary team is essential. Care is provided within an inpatient/outpatients setting through JH and JCC.
Qualifications
1. Master's Degree of Social Work from an accredited School of Social Work.
2. Eligibility for membership in the Ontario College of Certified Social Workers.
3. Recent (within 1 year) and relevant hospital experience with adult oncology/palliative care patient population required
4. Completion of palliative care certificate(s) preferred
5. Demonstrated skills in the performance of a therapeutic role including strong assessment, counseling, case management and advocacy skills.
6. Recent and relevant hospital experience with oncology/palliative care patient population.
7. Excellent communication skills.
8. Demonstrated ability in working as a member of an interdisciplinary health care team.
9. Crisis intervention skills.
10. Expert knowledge of community resources and the processes used to access those resources.
11. Ability to organize, prioritize and problem-solve effectively.
12. Awareness of and participation in psychosocial research activities.
13. Knowledge of the Health Care Consent Act and Substitute Decision Act.
14. Demonstrated skill and ability in the provision of health professional consultation and education i.e. Resident education, supervising social work students.
15. Experience and/or willingness to participate or take leadership in research and program and professional planning initiatives.
