POSITION SUMMARY:
The Social Worker plans, implements and evaluates psychiatric assessments and evidence based treatment. The Social Worker will provide individual and group treatment, with a particular focus in Dialectical Behavioural Therapy. The Social Worker promotes a psychosocial perspective congruent with social work training.
As a member of an interdisciplinary service, participates in formulation of individual and group treatment programs. Collaborates with other interprofessional staff and community partners to develop, implement and evaluate person/family centered programming. Maintains Standards of Practice as defined by the College of Social Work.
QUALIFICATIONS:
Clinical BSW, MSW preferred from an accredited University
Current registration with the Ontario College of Social Workers and Social Services Workers required
Membership with the Ontario Association of Social Workers preferred
Minimum of three years experience in an outpatient mental health setting
Demonstrated knowledge of clinical psychiatry and psychiatric medications
Demonstrated current knowledge of the signs and symptoms of mental illness, social work intervention strategies, and treatment modalities for clients with psychiatric disorders
Demonstrated crisis intervention, case management skills and knowledge of behavioral techniques
Demonstrate ability to develop and maintain effective professional and therapeutic relationships with clients
Demonstrated knowledge of relevant legislation which includes: Mental Health Act, Healthcare Consent Act, and Substitute Decisions Act
Demonstrated capacity to work autonomously as well as in collaboration within an interprofessional healthcare team
Demonstrated knowledge, training and recent experience in providing individual and group based Dialectical Behavioural Therapy in an outpatient setting
Demonstrated knowledge, training and recent experience in delivering individual and group based Cognitive Behavioural Therapy in an outpatient setting
Excellent communication, interpersonal, and problem solving skills
Ability to work effectively to ensure patient flow and effective caseload management within established targets
Demonstrated computer skills including use of Microsoft Office, Provider Portal, PHS, etc.
Excellent organizational and time management skills, including ability to prioritize workload
Excellent work performance and attendance record is essential.
RESPONSIBILITIES:
Develops and participates in the formulation of individual and group treatment programs
Engaged member of the consultation team through active participation and sharing of feedback
Maintains Standards of Practice as defined by the College of Social Work and working knowledge of relevant legislation (e.g. Mental Health Act, Healthcare Consent Act, Substitute Decisions Act ...)
Conducts comprehensive psycho-social assessments from a systems perspective
Identifies and intervenes with high risk client situations.
Provides evidence based interventions and counseling which may include crisis intervention, psycho-education, DBT, IPT, CBT, solution-focused therapy, therapeutic group work etc ...
Provides case management for an assigned patient group including facilitation of the client and team goals related to function and discharge, connecting the patient/family with services/resources, if necessary; coordination of all complex discharges and facilitation of specific case management activities
Provides formal and /or informal teaching of clients, families, staff, and the within community, as necessary.
Provides field instruction and supervision of Social Work students with affiliated university Faculties of Social Work.
Participates in program evaluation, Acute Mental Health committees, and research opportunities
Consults and collaborates with members of the multi-disciplinary team and with the community
The Emergency Department Admission Avoidance Program seeks to identify, strategize and implement discharge planning as soon as a patient arrives at St Joseph's Healthcare Hamilton. Admission Avoidance was adopted in 2016 out of great need to better serve the community demand for Emergency Care
The Admission Avoidance team allocates in-house, community and provincially-funded resources to get patients home sooner and straight from the Emergency Department
