Social Worker
Job Description
As a member of the healthcare team, the Social Worker provides direct patient care through the provision of psycho-social screening and assessments; individual and family counseling and treatment; crisis intervention; bereavement counseling; liaison and referral to community agencies; advocacy and discharge planning.Qualifications1. Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker
2. Baccalaureate degree in Social Work from an accredited School of Social Work, Masters degree in Social Work preferred
3. Previous crisis intervention experience
4. Expert knowledge of community resources
5. Demonstrated skills in the performance of a therapeutic role including strong assessment, counseling, case management and advocacy skills
6. Demonstrated strong interpersonal, teamwork and communication skillsPlease note that all Social Worker postings are posted at the Bachelor of Social Work (BSW) rate (unless MSW is required). Should the successful applicant possess a Masters of Social Work (MSW) and provide proof they will be adjusted to the MSW rate to correctly reflect the level of education in accordance with the letter of understanding.As a condition of employment, you are required to submit proof of full COVID-19 vaccination to Employee Health Services.
