Role Summary
The Social Worker is accountable to the Chief Executive Officer. They will foster a safe environment built on dignity and respect for our residents recognizing every resident is a unique individual.
Primary functions include planning and problem solving, holistic approaches to client care, conflict resolution, client advocacy, therapeutic relationships with clients and families, program planning, policy development, and interdisciplinary communication and teamwork.
The scope of practice will focus on the following: Social and emotional impact of physical and mental illness or impairment; Preservation and enhancement of physical and social functioning; Promotion of those conditions essential to assure maximum benefits from long-term health care services; Prevention of physical and mental illness and increased disability or dysfunction; and Promotion and maintenance of physical and mental health and optimal quality of life.
Qualifications
Bachelor of Social Work and current registration with the Nova Scotia College of Social Workers.
Certificate in Gerontology or Dementia Care.
Possess a comprehensive understanding of the characteristics of the resident population including Palliative Care.
Exhibit leadership qualities demonstrating creative planning for change and innovation.
Excellent communication, organizational, clinical documentation, and problem-solving skills.
Fit to work and provide evidence of good attendance and job stability.
First Aid/CPR certification.
Copy of all certificates to accompany application.
Excellent knowledge of Microsoft office including Power Point and presentation software/ equipment.
Performance Requirements
Ability to employ effective conflict resolution, mediation, and reconciliation approaches and techniques.
Strong interpersonal and communication skills.
Knowledge and ability to liaison with community resources available for client/family support and related health services.
Ability to promote staff morale, cooperation, assertiveness, creative planning for change and innovations.
Experience in development and implementation of policies, programs or other protocols.
Able to work independently and collaboratively with other members of the multidisciplinary team.
Advocate on behalf of residents with respect to issues like Powers of Attorney, Substitute Decision-Makers and other legal issues.
Demonstrated ability to promote resident-focused care that demonstrates care for and with residents and significant others, sensitive to diverse cultures and preferences, social justice concerns, and resident advocacy.
Uphold provincial and national standards of practice and code of ethics.
