Job Description
Summary of Duties
To provide a full range of social work services to patients and their families admitted to the Trauma program. This includes psycho-social assessments, counselling to patients and families, discharge planning, crisis intervention, linkage to community resources, team consultation.
Qualifications/Skills
Masters of Social Work Degree Required Member of the Ontario College of Social Workers and Social Service Workers in Good Standing Minimum two (2) years of experience working in a Hospital setting Experience in Trauma preferred Exceptional communication skills required Knowledge of the HCCA and SDM act In-depth understanding of the Sunnybrook Policies that impact transitional planning Strong negotiation and conflict resolution skills Demonstrated skills in Interprofessional Collaborative Practice & Education Ability to balance collaborative practice with self-direction Ability to manage multiple tasks and set priorities in a fast-paced environment Commitment to student education Experience and commitment to quality improvement initiatives * Demonstrated knowledge of organizational priorities and social work scope of practice priorities
