Job Summary

We are seeking a compassionate and dedicated Social Worker to join our healthcare team. The ideal candidate will provide comprehensive support to patients, families, and caregivers, particularly within pediatric settings. This role involves coordinating care, offering emotional support, and ensuring patients receive appropriate resources to improve their well-being. The Social Worker will play a vital part in fostering positive patient outcomes through case management, research, and patient advocacy.

Responsibilities

Assess the psychosocial needs of patients and their families, especially those involved with pediatrics and special education programs. Develop individualized care plans to address emotional, social, and practical needs. Collaborate with medical teams to coordinate patient care and facilitate communication between healthcare providers, patients, and families. Provide counseling and emotional support to children and their families coping with medical conditions or hospitalization. Conduct research related to patient care strategies, community resources, and best practices in social work within healthcare settings. Manage case documentation accurately, including data collection and maintaining detailed patient records. Assist with discharge planning by connecting families to community resources, support services, and educational programs. Advocate for patients' rights and ensure compliance with healthcare policies and regulations.

Qualifications

Proven experience working with children in a healthcare or social services environment. Prior hospital experience is highly desirable. Knowledge of medical terminology relevant to pediatric care and patient treatment plans. Strong background in case management, patient care, and data collection methods. Familiarity with research techniques related to social work or healthcare practices. Experience working with special education programs is a plus. Excellent communication skills with the ability to provide empathetic support to diverse populations. Ability to work collaboratively within multidisciplinary teams in fast-paced settings.
