Reporting to the Manager of Social Work, the Social Worker provides client?centered psychosocial assessments of
the emotional, physical, cognitive, and social needs of patients and families within the Cardiology, Acute Stroke, and
General Medicine inpatient units, as well as the Cardiac Rehabilitation outpatient department. The Social Worker
advocates for patient and family needs while assisting with hospital discharge and care planning. This position
functions within a multidisciplinary team and adheres to the standards of the Ontario College of Social Workers and
the Thunder Bay Regional Health Sciences Centre's (TBRHSC) Mission Statement and Code of Conduct.
EMPLOYMENT REQUIREMENTS:
Education/Experience: Bachelor of Social Work (BSW) degree
Current unencumbered registration with the Ontario College of Social Workers and Social Service Workers
Minimum two years of experience in an acute care environment, preferably working with patients with cardiac
disease, acute stroke, and complex medical conditions
Skills/Abilities: Strong assessment and counseling skills with individuals and families
Ability to work effectively in a fast?paced and evolving health care environment
Knowledge of chronic disease management, particularly cardiovascular diseases and complex medical/social
issues Familiarity with transportation options in Northwestern Ontario, including the Northern Health Travel Grant and
Hope Air Understanding of relevant legislation, including the Health Care Consent Act, Substitute Decision Making, and the
role of the Public Guardian and Trustee Excellent verbal and written communication skills to engage with patients, families, health care professionals, and
community stakeholders Competency in conflict resolution, problem?solving, and critical thinking
Ability to adapt interventions and care plans to rapidly changing patient conditions
Proficiency in coordinating and facilitating family meetings
Knowledge regarding funding for special equipment, medications, and community resources such as addiction
services, mental health services, and crisis intervention
Assets: Experience supporting patients and families with end?of?life care and bereavement
Familiarity with community resources specific to cardiology, stroke, and general medicine in Northwestern Ontario
Previous experience working within a hospital?based multidisciplinary team
Experience in teaching acutely ill patients and their families
Salary Range: $39.160 to 50.770
