Position Overview
Under the direction of the Lead, the incumbent works as part of an integrated clinical team within Complex Community Service Integration (CCSI). The incumbent's role supports the Community-Emergency Department Violence Intervention Program (CEDVIP). CEDVIP supports youth (ages 14-29) identified in hospital Emergency Departments who experienced injury related to violence to receive wrap around supports utilizing the Circle of Courage model.
The incumbent will provide professional, skilled psychosocial assessment and counseling interventions to youth, families and groups; providing a range of psychosocial interventions that are aimed at assisting youth, families and groups to cope with and adapt to health and social related issues, using the healthcare system and other social systems effectively and efficiently; demonstrating leadership in assisting the healthcare team to provide care and complete transitions; demonstrating proficiency in trauma-informed care, person centered planning, harm reduction; and integrating cultural teachings and health practices into care plans.
Participating as a member of the healthcare team interprets social, psychological, emotional and family problems to the team while maintaining excellent working relationships and participating in team planning and problem solving; Provides clinical consultation support to team members on support approach and performs a variety of administrative functions as required by the department. May be requested to assist in providing instruction to students and new staff.
